Graduate Credit Registration Procedure

Instructions

1. Complete this form, print a copy now, and mail the printed copy with the check for your kit(s) to the address listed below either now or after you register (Step 4). After printing, please submit a copy electronically by clicking on the "Submit" button.

2. Apply to the Graduate School at the University of Kentucky (unless you are already a UK graduate student). This m ay take approximately 2-3 weeks.

3. After you receive notification from the graduate school that you have been accepted, you may register for the course(s). Register with the University of Kentucky for PHY401G Special Topics in Physics and Astronomy, and pick the section(s) (Light,Temperature & Heat, Electricity & Magnetism, Force, Motion & Energy) you would like to take.

*Please note: You will be billed for tuition by the University of Kentucky. Do not include tuition payment with this form.
